SCHEDULING AND FEE CHARGES
Don't disrupt your normal routine or life; scheduling is easy, convenient and flexible. At Keyes to Wellness, evenings and weekend appointments are available.
As the cost of healthcare continues to grow, I have set reasonable fees so that I am competitive with other providers and most importantly, so that rates are affordable for my patients. By not accepting insurances, I am able to keep both my costs as well as your fees at a reasonable rate by not having the additional overhead required to collect payments from the insurance companies. The service fees are listed next to the appointment type:
- Initial Psychiatric Evaluations $300 to $450
- 45 – 60 Minute Therapy Session with Medication Management $225.00
- 30 Minute Therapy Session with Medication Management $125 to $150
- Dictated Letters $35 to $50
- Dictated Reports and/or Evaluations $100 to $150$
**No Shows & Cancellation of less than 24 hours' notice may be subject to the Posted 50$ Service Fee.**
Insurance
Insurance policies are not accepted at Keyes to Wellness, however, numerous insurance companies offer out of network benefits after a deductible is met. This return benefit may range from a 70 – 80% return in the monies that are paid by the patient. An invoice identifying the CPT code, description of service and cost of the visit will be provided to you as a receipt. The invoice/receipt can then be submitted to your insurer to meet your deductible and/or obtain reimbursement for my professional services.
Payment
Payments for sessions are due in full at the time of service. Cash and checks are accepted as a form of payment. Applicable fees will be charged for any returned checks.
